There are a couple of possibilities:

Jether also went by the name Ithra and had dual citizenship or mixed nationality as an Israelite and an Ishmeelite (which is perhaps why he went by two names---perhaps he adapted his name after moving his citizenship).
- Jether is the biological father, but Ithra is the step-father
